Guidelines to submit your case

1. Complete history of the patient (The cases should be submitted as word document.). The name and contact details of the patient to be excluded to maintain privacy of the patient. However if the patient agrees a written consent along with his testimonial can be recorded (written or video).

2. Submit at least 1 year follow up with complete details as to prescription and other auxiliary treatment used if any.

3. Well documented investigation reports before and after treatment (X-rays, Laboratory investigation, M.R.I., CT-scan, photographs, videos, etc.). You can scan the reports and upload them. Even videos of patient if taken can be submitted.

4. A brief bio-data, contact details and your passport size photograph should be included. All cases published will contain an acknowledgment about your contribution.

5. All rights for publishing the cases are at the sole discretion of Hompath. The cases may be published on websites, print media, knowledge software CDs, and other mediums as judged appropriate by Hompath.
