ASTHMA: MIASMATIC UNDERSTANDING AND TREATMENT

Selection of the best remedy and effective treatment for asthma has left many seasoned professionals nonplused in their efforts to bring relief to their suffering patients. Only after understanding the disease, it can be managed easily.

To obtain a clearer understanding of asthma, it’s best to know the structure and function of the upper and lower respiratory tracts. The case taking can then include collections of relevant and important information on symptoms, signs, clinical categories, epidemiology, patho-physiology, investigations, differential diagnosis, staging, and complications related to the case. These are the most important aspects intimately connected with the management of asthma.

Management of asthma can be broken down into disease factors, patient related factors and management of the case as follows.

Disease Related Factors

  1. Intensity of illness
  2. State of patient at time of diagnosis
  3. Pathological changes.
  4. Miasmatic stage and classification
Patient Related Factors
  1. Age
  2. Previous treatment
  3. Drug dependence, steroid and other medications
  4. Maintaining causes
  5. Associated complaints and complications
  6. Cooperation of patient and relatives
Management
  1. Selection of homeopathic remedy
  2. Potency and repetition
  3. Diagnosis of synergistic action of other medicine (s)
  4. Diagnosis of antidotal action of other medicine (s)
  5. Devising ancillary measures such as
    1. Diet
    2. Exercise
    3. Yoga

For better understanding, let us take the examples of different Miasms.
When the Patient Reports in the Psoric Phase:

  1. He will show a typical psoric personality including excitability, irritability and erratic nature.
  2. He will have an acute skin complaint or a skin complaint alternating with the respiratory complaint.
  3. He will have some strong psychological symptoms or some maintaining psychological cause, which produces asthma.
  4. Any measure, which re-establishes the skin symptoms, relieves asthma soon.
  5. The patient will have more spasms and minimal secretory phase. Breath holding spasms in children are also covered under this category.

Management of Asthma in the Psoric Phase:

This will require a homeopathic simillimum in a higher potency with minimal repetition. As soon as the skin symptoms are reestablished, medicinal treatment should be immediately discontinued.
Usually in cases in the pasoric phase, no acute remedy for asthma or skin is necessary. With the repetition of a placebo, control over the disease is maintained. If skin symptoms improve during the treatment but the patient starts getting repeated attacks of cough, cold, and sinusitis, the case should be taken again. Perhaps the treatment was wrong. Reevaluate the patient’s history and select a different remedy.
Occasionally, when there is an acute skin condition, the patient may need an acute remedy. Otherwise he won’t come out of the crisis. Once the crisis is over, the patient will recover fully.

When the patient reports in the Sycotic phase, he will have the following symptoms:

  1. Sycotic personality i.e. sluggishness, slowness, erratic response and behavior, responding to various allergens in the atmosphere, etc.
  2. Aggravation of the condition will be caused by the rainy season and also by getting wet. If so, the patient feels much better in a dry climate.
  3. There is marked secretion with great difficulty in expectoration. Expectoration in greenish, yellow, and thick in nature (characterizing typing sycotic inflammatory discharge).
  4. 4. It is likely that the patient has had a past history of suppression of skin complaints and discharge from the mucous membrane.
  5. There might be concomitant musculo-skeletal manifestations.

Management of Asthma in the Sycotic Phase:

This will require frequent repetition of acute, constitutional and intercurrent remedies in low or medium potency. Reestablishing discharge from the mucous membrane will immediately relieve asthmatic symptoms. The response in such cases is usually gradual. The patient recovers totally and he will not have a recurrence of an attack. In such conditions, a high potency is likely to produce an aggravation. It may also produce medicinal symptoms. If the treatment is correct, the patient goes to the psoric and usually does not require any further remedies. If at all required, it may be the same remedy in a higher potency or a complementary or cyclical remedy.
If the case is mismanaged, the disease and the miasmatic condition progress rapidly to the tubercular phase. The patient may go directly into the tubercular phase, skipping the sycotic phase, if he has a strong past or family history of tuberculosis or because of strong suppression of the disease condition.

The Tubercular Phase will have the following.

  1. Unpredictable attacks and rapid changes in severity of the symptoms.
  2. Patient might have a history of viral infections leading to asthma.
  3. He will show tubercular personality, which includes being lean, thin and is easily exhausted.
  4. He will have a cough with excessive expectoration, which is purulent, muco-purulent and bloody.
  5. Patient will have easy suppression with tendency to infection and fever.
  6. There will be fibrosis of the lung tissue associated with asthma, complicating respiratory functions.

Management of Patient in Tubercular Phase:

This is always difficult as the response to the treatment can be as erratic as its nature. The doctor will need to make a careful remedy selection, potency and repetitions. With this type of patient, give an acute remedy in low or medium potency to prevent the recurrence of symptoms.
Constitutional medicine should be in medium or higher potency so that the patient recovers faster. Very few patients in this advanced stage recover totally and remain symptom free throughout their life. For most of the patients, we may have partial relief with a high probability of recurrence and repetition of attacks. This type of patient is difficult to manage if there is an additional infection. The treatment will include higher potency with frequent repetitions until there is a response. As soon as the patient responds, the medicine should be discontinued.

Syphilitic Phase of Asthma:

In this phase, there is already structural damage producing emphysema and other complications. It is best to give an acute remedy or partially similar remedy or partially similar remedy only. Totally similar remedies may produce severe aggravation and other complications, which we may not be able to handle. Cure in these cases is out of question. Only palliation can be achieved.
This type of miasmatic understanding and differentiation helps in managing patients more easily and effectively.
If you follow this chart mentioned below carefully, you can manage nearly 70-80% of the patients in your practice.

This chart is created with a link between remedies, which are connected by lines. The connected remedies are related to each other. Also they have to be differentiated before we decide about Simillimum. In addition, I have given salient indications of each remedy derived from practice. If you get these indications in the patients, you have good chance of recovery even in advanced cases.
